Hỏi bệnh sử tốt là bước đặc biệt quan trọng đầu tiên để chữa bệnh bệnh nhân tất cả kết quả. Mỗi y/bác bỏ sĩ sẽ sở hữu được một phong thái cùng chuyên môn riêng biệt nhằm hỏi người mắc bệnh nhưng lại để có một cuộc vấn đáp giỏi đòi hỏi y/bác sĩ cần triển khai các phần cơ bản sau:
I. Các bên trong một bệnh tật y khoa
Để đạt được một bệnh tật ghi bằng tiếng Anh sau phần kính chào cùng ra mắt của chưng sĩ tất cả các phần sau:
1. Các chi tiết cá thể (personal details)
2. Lời knhị bệnh/nguyên nhân nhập viện (present complaint)
3. Bệnh sử (past medical history)
4. Tiền sử sử dụng dung dịch (drug history)
5. Tiền sử bệnh dịch vào mái ấm gia đình (family history)

8. Xem xét những bộ phận với các cơ sở (review of the system)
II. Các thắc mắc của bác bỏ sĩ cùng với bệnh dịch nhân
1. Các thắc mắc thường xuyên gặp gỡ vào phần cụ thể cá nhân. Phần này gồm:
-Câu hỏi về tên tuổi, chiều cao cùng trọng lượng

Bác sĩ thường xuyên hỏi về triệu triệu chứng (symptoms). Đau (pain) là một trong những trong số những triệu chứng phổ biến duy nhất. Glendinning & Howard <1> nêu ra 12 điểm đặc trưng về nhức cùng khớp ứng với những đặc trưng sẽ là các các loại câu hỏi tiêu biểu vượt trội.

b. Sự lan tỏa (radiation):

e. Thời gian ban đầu (Time of onset):

h. Nhân tố làm cho tăng đợt đau (Aggravating factors):

Do you feel anything else wrong when it’s there?

Có thể sử dụng tự tắt (acronym) SOCRATES nhỏng một cách lưu giữ bổ ích nhằm hỏi về triệu bệnh đau
Site (vị trí): Where exactly is the pain?
Onset (bắt đầu cơn đau): Where did it start?
Character (sệt điểm): What is the pain like?
Radiation (sự lan tỏa): Does it radiate/ move anywhere?
Associated features (đặc thù kết hợp): Is there anything associated with the pain e.g sweating, vomitting?
Time course (khoảng tầm thời gian): How long did it last?
Exacerbating/alleviating features (đặc trưng làm tăng/giảm cơn đau): Does anything make it better/worse?
Các thắc mắc về phần bệnh dịch sử rất có thể thực hiện bằng:

a. Chi ngày tiết cần sử dụng thuốc (details of drugs and medications):

Are you taking any over-the-counter (OTC) medications?

Các câu hỏi ở phần này thường xuyên liên quan mang đến tuổi tác, sức khỏe hoặc nguim nhân chết của bố mẹ, anh bà mẹ, vk chồng cùng bé cái

6. Các câu hỏi hay chạm mặt trong phần tiểu sử từ trước cá nhân và làng hội
Các thắc mắc ở trong phần này thường tiềm ẩn những thông tin về nghề nghiệp, cửa nhà, kinh nghiệm cá thể nhỏng vui chơi giải trí, thể dục, thuốc lá cùng rượi, trường hợp là trẻ em, các ban bố về ngôi trường học với mối quan hệ vào mái ấm gia đình. Sau đó là các câu hỏi tiêu biểu:
